Client’s Initial Assessment

At Golden Grace Senior Care, we begin by collecting essential information to better understand how we can provide the best possible care for you. After gathering your basic details, we offer a complimentary in-home visit, during which we conduct a needs and environment assessment. This helps us gain a clear understanding of the support you require and how we can best assist you in your home.

Client-Centered Plan of Care

Once we've completed the initial assessment, we will create a personalized Plan of Care tailored to your specific needs. This plan will take into account your loved one's history, daily routine, and preferences to ensure the highest level of support. We will review the care plan with you in detail and make any adjustments necessary to ensure it perfectly aligns with your expectations and requirements.

Identifying the Right Caregiver

Welcoming someone into your home is a big decision, and we are truly honored by the trust you place in us. With this responsibility in mind, we make every effort to match you with a caregiver who possesses the skills and qualities that best meet your needs. All of our caregivers undergo thorough screenings, including background checks and reference verifications, and are fully bonded and insured. If for any reason you have concerns, we are more than happy to arrange for a new caregiver to ensure your complete comfort and satisfaction.

Ongoing Support

Our caregivers complete detailed visit notes after each session, which help us track the progress of care and ensure everything is going as planned. Additionally, our Care Inspector regularly visits clients to ensure care is being provided according to the established Plan of Care. If any changes are needed, our staff is fully equipped to make adjustments to the care plan, ensuring that your loved one always receives the support they need.
